On one of my systems, the manager can no longer connect. This happens on versions 6.6.20, 6.6.23, and 6.6.28. I'm running it in single user mode now, however, I had previously tried and then backed out of (ie uninstalled) BOINC as a service.

I'm guessing that I've been hit by a firewall issue at work because the single user mode seemed to work for about a week after experimenting with service mode. Everything stopped working after my system was rebooted. What local port(s) need to be open? And, is there some 'residual' (e.g. maybe a regedit parameter) that might be confusing the manager between service and single user mode and causing this issue?

You'll have to make sure that you set the following allowances in your firewall software:

- Boinc.exe and Boincmgr.exe are allowed to talk to each other on TCP port 31416.
- Boinc.exe is allowed to make a connection to the internet on TCP ports 80 and 443.

If I have access to a proxy server, do I still need other two ports (80 and 443)?

No, you then set up the proxy server in BOINC Manager->Advanced View->Advanced->Options.

Same with me after upgrade from 6.4.7 to 6.6.20. Had to revert to the older version.
