Both Trac and MediaWiki ask you for a one-line summary of your edits when you are submitting changes. In Trac this is totally optional, while MediaWiki will prompt you for it if you forget (this is configurable in preferences). Even so:

It is really useful if you write a short description of your changes every time you submit to the wiki
